What about turning some on a lathe?  Chuck a hard-rubber or soft-plastic 
bit of stock, turn the OD, a flat face, and hollow the face down to the 
needed depth (for the snap-on side).  Then make a little tool to cut the 
groove on the inside.  Then just part it off (at the hammer-side) to the 
proper thickness. Maybe?
