Not all 28 RO covers have the middle hole.  Some have two screw holes  
for a circuit number label.  If my memory serves the middle hole is  
for a BREAK key or could also be used for mounting a toggle switch  
for whatever...  Most of these I have seen simply have a blank  
installed.
