Now that the problem seems to be narrowed down to 3rd pulse, can you check  
to see if either the code bar immediately above or below are interfering with  
the code bar for the 3rd pulse.
 
Here is a quick test that might help find out...
 
Line Feed is 2nd pulse only...
 
Carriage Return is 4th pulse only...
 
See if it makes a difference if you type ( or make a paper tape) of the  
alternating sequence of the letter O then LF then O then LF... or O then CR then  
O then CR.
 
Hopefully, this will help pin down the problem a little  further.   I saw a 
similar problem where there was a slight  interference between two code bars, 
causing the gain of a pulse...
