If your Model 28 makes an O an M, then you are gaining a 3rd pulse.
 
O is pulses 4 and 5.
M is pulses 3 and 4 and 5.
 
You might also have the letter T becoming H.  and the letter W  becoming Q.
 
If you can confirm this, then you can work to a little bit more to see  if 
there is a bind on the mechanics that controls the 3rd pulse.
 
A couple of other thoughts, do you happen to know the range of the Line  
Printer?  Has the Line Printer been cleaned and lubricated recently? 
 
I took my Model 28 ASR out of storage (10 years) about six months ago and  
found a number of the problems disappeared with a good cleaning and lube.
