It's the responsibility of the party using whitelists (in this case, the
replying party) to ensure that the responder will already be whitelisted, or
they don't DESERVE a reply.
No spam from earthlink here, they go straight to trash.
Whitelists do work, but it's not my responsibility to maintain my records on
your whitelist.

You can consider it to be anything you want.
You can choose to block everything from every ISP (AOL is a better taget,
imo). Just think of how much email you won't have to receive!

There's a lot to be said for a whitelisting policy. It sure as hell is
effective - much more effective than most other methods.
It also serves to provide the person using it with a greatly pared down
inbox, largely because of people who go insane when whitelisting is
implemented.
