Rod, K0EQH hit the nail on the head:
I need one like the Switchcraft # 14B. However, it mounts in a 3/8" hole, 
not 1/2".
For those  of you who might not be familiar with Switchcraft, here is the 
description:

Female chassis mount, 3/8" hole.
Three circuit, two closed circuit.
This jax is what you would plug a set of stereo headphones into and it has 
the two extra contacts on the tip and ring.
I have a good pic I can send you (Reflector will not allow attachments so I 
have to send it to you direct).
