[FCARC] W8TUX Contact Information Release

As discussed at the meeting, W8TUX is coordinating with our County 911
Director who has asked for emergency contact information for the various
club members who participate in emergency service activities.  We are
asking all who participate in ES activities to please indicate you are
authorizing us to release your contact info to the 911 coordinator. The
purpose is to expedite contact out to all radio amateurs who might be able
to assist in an emergency, it does not obligate you, but is to inform you
help is may be needed.
