Recall: S and H Products - Hoseline Tee with Valve

A manufacturing error in certain hoseline tees with valves was noted by the
field, reviewed by the San Dimas Technology and Development Center, and
confirmed by the manufacturer. The error resulted in a percentage of
hoseline tees with valves manufactured by S and H Products that do not have
the requisite number of threads on the valve stem. For affected tees,
rotating the valve handle to a fully closed position causes the threads on
the valve stem to run out and disengage with the valve body. This does not
allow the valve to be easily reopened and constitutes a performance defect
and possible safety hazard. (PDF, 129 Kb)
