Probably around '89, when the home we bought in the Santa Cruz Mountains 
15 years later was rebuilt, a telco installer ran the "ground" for his 
drop on about 50 ft of #10 to the outlet for a garden hose, which was 
fed by PVC pipe. The only earth connection was outside the kitchen of 
the detached "mother-in-law" garage apartment (now my shack). The bond 
from it to the panel in that building was #10 running up through the 
ceiling across the building, down the outside wall, to a single driven 
rod. I also found 120V outlets in that kitchen wired between one leg of 
240V and the green wire.

Luckily, I didn't have lightning damage before finding these issues and 
fixing them.
