I ultimately went to the Boss FS-5U as well.  My guitar player used the same one for years and I figured if that dude couldn't break it, it would be good enough for me.  Normally Open.  No modification.  I love mine.

I modified a Boss FS-5U foot switch for this purpose. It has served me well for years.
