Since I unleashed this discussion yesterday, I've had a lot of interesting
and enjoyable off-list correspondence that, I believe, uncovered the issue.

When I last worked in non-Amateur voice communications over a decade ago,
headphones weren't common unless one was in a very noisy environment such as
an airplane cockpit.

Headphone with integral micas have become very common today in both Amateur
and commercial use, which makes sidetone not just practical, but preferable.

And all the folks who pointed out that the level of the sidetone can be used
to greatly reduce the tendency to shout are correct. I've seen that
demonstrated with good effect to quiet down a "shouter" in the office by
turning up his earphone volume without his realizing it.

In the controlled environments of which I am familiar (military and
commercial) all operators have side tone whether they like it or not. We
amateurs have a choice of side tone or not and I'm coming to the conclusion
that's at least half the problem of shouting.
