I had this problem and the issue was the switching
power supply.  I installed a different power supply
and it was fixed.

Also, the problem could be radiation from cables that
come out of the computer.

One suggestion I was given is to try disconnecting all
cables (other than power) and see if the noise is from
the case itself.
