[Elecraft] Help Voltage check KPA100

I am at the voltage check on page 43 of the KPA100 manual. I only have 0.5 V
on the cathode of D13 instead of 90 to 150 V. 

I have listed some other voltages I have measured. Some measurements were
different at different times I measured them. They are marked with an
asterisk.

I redid T3 very carefully with no effect. 

What should I look for?
