Thanks for the suggestions.  

VRFDT on Pin 6 of U6 is sat at 0.01v and VRFEN on Pin 8 of U6 is sat at
5v.  

I did try increasing CAL CUR up to 4A, but it made no difference.  As
soon as the PA is connected, the HiCur warning appears and the output
from the basic K2 gets inhibited. 

I guess I have a problem around U6 or Q10 and Q11?  I'm pretty sure
VRFDT should not be 0v.
