I don't start in the middle, for the reasons Don mentions, but I do "throw
away" the first turn, sometimes two. That is, I start winding from one end
and, sure enough, the first turn or two is sort of sloppy because I'm
holding onto it while I wind. So I just keep on winding until I have one or
two EXTRA turns. Then I CAREFULLY count and recount the turns to be sure I
know how many I have, and unwind the excess from where I started. 

Now my "first turn" is as tight and straight as the rest.
