I can't tell you what the original size is/was, but I had the same type of 
issue.  I don't recall if the hole itself was too small for the connector, 
but the hole-to-jack alignment wasn't quite right, giving the same 
result.  I took a drill bit slightly larger than the hole and gently reamed 
it out by hand to its present diameter of 10 or 11mm.  As I mentioned, a 
slightly smaller hole would work as well if the alignment were better. The 
few other fixed jacks all lined up just fine.

OBTW, my rig is # 183.
