[EIDXA] Registration form for Aug Cedar Rapids convention

I have been asked for the registration form for the upcoming ARRL Midwest 
Division Convention.
This is sponsored by the EIDXA, CVARC, & CARC clubs.

The convention will be Friday August 5 thru Sunday August 7.  See the link below

The convention forums are still being finalized, but we plan to have DX forums, 
Youth forums, and technical topics.  Check the website for the latest details.
Ed Hare, W1RFI from the ARRL will talk interference, And ARRL President Kay 
Craigie N3KN will be the banquet speaker.
Seating is limited.  The registration form is attached.

I plan on being at the meeting this week, and will accept reservations in person 
for early birds.
