[Dxbase] New colorized matrix report - LOTW and CARD cfms - Entity vs Band vs Mode

ALL DXbase users:

I have made a brand new " 5 Band - 10/15/20/40/80 " color matrix report (standard style) that shows

ALL entities - by Band and by Mode -- for LOTW cfms and separately for CARD cfms

I have setup a webpage for this report, with instructions on how to use it and
LINKs for viewing an example output in 3 different outputs (so that you can see
what the report will look like).
