[Dxbase] Prefix mapping---one more time.

I am still using DXB2005, which except for one issue suits me fine.

That issue is the overly complex maintenance of the prefix database.  
When and if a new version is offered that automates this with a list 
maintained by "headquarters" I will be first in line to upgrade. (Hint)

In the meantime, I seem to have my ref database totally hosed up with it 
showing 340 current countries, which is of course two too many.  I 
suspect this is due to my past attempt to fix the Cook Island fiasco.

I know this has been discussed in the past, with various opinions 
expressed, but I'm looking for a "final" version.
