[Dxbase] Update to Joe WA6AXE's setp guide

To all:

I just noticed that when I copied and pasted the Primary Prefix
database for the 4O entry -- the "normal" entries for some
columns were shown as 0 and -1 ... so, here is the final 4O entry

Please NOTE:  I modified DXbase 2007 mdb files .. If you are using
2005 or earlier, the TimeZoneDiff column will NOT be there. Just ignore
the  +0100 ...
