[Dxbase] New entity #338 setups for the Prefix Tables - Joe wa6axe

Thanks for the info Joe on the FJ addition to DXBase.  I did everything by
the book and found it would not save the new FJ entity.  Problem is the
ARRL-ID.  In my database, 516 is assigned to 40 Montenegro.  Any ideas.
