[Dxbase] Printing QSL Info Directly on the Card

First off, apologies because, in all likelihood, this has been 
answered numerous times before. I've been on the list a long time, 
but recently changed things around and lost my local archive. That 
said, my question is as in the header: would any of you point me to 
how I can design a "label" or form that essentially prints directly 
onto my QSL card, instead of onto a label that I stick on the QSL 
card? Since every card format is a little different, I expect to 
experiment to get it as I want it.
