[Dxbase] LOTW WAS Report

Joe,

Thanks for making the report for LOTW WAS so fast. I still need 6 states 
on LOTW. I thought that working the ARRL SS for a few years in a row would 
have done it. 

I think it would be nice for DXbase to list ALL states on the 
OUTPUT/AWARDS/WAS forms.  The ones not confirmed would have a blank line 
after the state name.
