DXBase 2005 addresses the issue of multiple qso type cards.
You enter a UNIQUE number in the qsl# field for each multiple qso card.

as for your second question.
It makes no matter whether you submit cards or LoTW credits.
The fees are the same, you may save a touch on postage.
The NEW downloadable ARRL DXCC submission form has a box to check if you want LoTW credit
included in the submission.
Also on the LoTW site, in the submission process, it asks if you wish to combine these
with a hardcard submission that you are or have submitted.
