When you go to Download Report in the LOTW, the date in the "Show QSLs 
received since:" auto-fills with the date of your last LOTW report 
download. In theory, all the LOTW QSLs before that date should already 
be confirmed in your log, and all the QSLs after that date should be new 
confirmations. In practice, however, somehow that doesn't work 100% and 
there is a slight overlap. I don't know why. It could be due to the 
differences between Universal Time, ARRL time, and your local time zone, 
but I'm guessing.

I usually backdate the default date by a week and that works out fine. 
DX4Win ignores the already confirmed QSOs anyway and you're good to go.
