Larry K5RK and I are using the same version of Firefox: 102.0.1, 64 bit
My version of Win10 is slightly newer: Version 21H2; build # 19044.1826
Installing the Content-Type Fixer in Firefox cured the problem for me.
Thanks to ON5GM.
None of this explains why the ADI extension is stripped when downloading
from LoTW but not from Clublog or other sites.

Agree, and the error is also in Windows 11 and Firefox version 102, 64 bit.

This appears to be a problem with the way Firefox and LoTW talk to each
other.
First thing I checked was to make sure the Windoze setting "Hide extensions
for known file types" was still unchecked. It is.
Downloading an .ADI file from Clublog shows the .ADI extension.
Downloading an .ADI with MS Edge (Win10) or Safari (IOS) shows the .ADI
extension.
Downloading files from other sites with Firefox shows the proper file
extensions.
Now looking at Firefox release notes and settings to see if I can find out
what has changed. This just started with a recent Firefox update and I
cannot figure out how to go back to an earlier version of FF for testing.
In the grand scheme of things, it's trivial to type in .ADI when downloading
the report. The trouble is remembering to do it.
