[Dx4win] Export to special ADIF

Hi all
It looks like my log in JTDX is corrupt. So I would like to export my 
DX4Win log to adif format, but only those  fields that are stored by the 
normal operation of JTDX.
These are call, gridsquare, mode, rst sent, rst recvd, qso date, time 
on, time off, band, freq, (my) station callsign, my gridsquare.

Any help on how to achieve that please ?
