[Dx4win] FT4 import to DX4WIN

Hi,

ADIF release 3.10 (http://adif.org.uk/310/ADIF_310_annotated.htm) now 
includes FT4 as a submode of MFSK. As FT4 is increasingly found on the 
bands, a corresponding extension in DX4WIN would be quite nice ... (and 
also for JS8, which is also defined as MFSK submode).
