[Dx4win] Deleted Countries

Kingman (and all other deleted countries) still count towards your mixed,
phone, CW, rtty, and individual band DXCC totals.  They do not count for the
Honor Roll or the DXCC Challenge.

Unless I'm missing something, KH5K is still Kingman reef, it's just no 
longer a "counter" for DXCC purposes.
