[Dx4win] ADIF to DX4WIN log conversion version 5.21 released

I have released version 5.21 of ADIF to DX4WIN, you can download it from here:

      Added Idaho QSO Party

This state was only tested with a TR4W log.  N1MM probably works, not
sure about WriteLog.
