[Dx4win] Removing Duplicate QSOs

I have written a new application note on removing duplicate QSOs from
your logbook:

I have to check with Paul on a couple things, but I tested the
procedure and it does work.

p.s. the app. note is missing the menu on the left (that the other
app. notes have), I have to use a different editor to fix this, and I
don't have that editor installed on this computer. I should be able to
fix it Monday.
