Use the form at http://www.rickmurphy.net/lotwquery.html

Enter your LoTW account callsign in the "LoTW Username" box and click
"Submit". That will get you an ADIF file with all of the QSOs that you
have uploaded to LoTW.
Note that LoTW doesn't keep a lot of QSO details - callsign, band,
mode, frequency, date/time is about all you can get in this manner,
but it's better than nothing.
