[Dx4win] DX4WIN.CTY Release #62 - first call

I have started working on DXWIN.CTY #61.  You can read the release notes here:

Please let me know if you have any additions or corrections.  This
will be a quick release because I want to get it out on 1 June 2009
due to some prefix changes for the Azores.
