[Dx4win] DX4WIN.CTY Release #57 - first call

I have started working on DX4WIN.CTY Release #57.  You can find
preliminary release notes here:

I wanted to get some early notes jotted down before I forgot them.  I 
plan to release the new country file the weekend of January 10/11, 
sooner if I can squeeze it in (but not before the new year).  If you 
have any additions/corrections, please let me know.
