[Dx4win] New application note: Adding a New Entity

Dear Jim,

You do not need to appologize for your greate work you are doing keeping up 
the CTY file and all other files conected to DX4WIN.

While writing to you I would like to wish you alot of good dx and a lot of 
new contacts on the bands.
