[Dx4win] Adding QTH info for Direct QSLs

To print the QSL manager's address label the QSL route must be "Direct", 
but if the QSL manager is the same station, then "via ..." is omitted on 
the QSO label (very nice!).  Of course, you have to check "Print QSL 
managers" on the printing dialog.
