[Dx4win] Eliminating Mgr address labels, but keeping mgrlisted on QSO label

Even i would like to have a check box for this, i think that the address is 
printed only when i have checked the qsl for direct. Use another option for 
the direct cards and print them, until Paul fix it. It is very difficult 
indeed when i have to print direct in qsl cards.
