[Dx4win] Updated "Working with LoTW" application note

I made a few updates today to the "Working with LoTW" application note:

1.  Added a section for checking your log before trying to export 
(avoid problems with PSK).

2.  Note that only QSOs with Upload="Y" will be exported.

3.  Describe what to do if an error occurs during TQSL signing.
