Short answer: you can't import "extra" data from the LoTW report file.  Paul may add this in a future version.

If you use the "New QSO over-writes existing" you should get the extra data, but you might lose other info you logged with the QSO.

Only the first option you tried will set the confirmed flag.  The "overwrites existing" option is generic for ADIF files (i.e. importing from contest logging software), so in general you do NOT want this changing confirmation flags.
