[Dx4win] Notes For This Call Oddities

I just did some more checking and found that ALL of my Notes For This Call 
entries have been duplicated.  I don't relish having to hand-fix a thousand 
or more entries, but I sure would like to ensure that it doesn't happen any 
more.

Help!
