[Dx4win] Release 12 of DX4WIN Country File available

Hi Folks,

Release 12 of the DX4WIN country file is ready. This is the version that 
will be shipped with DX4W604. To get it early, you can download it from:

   http://www.ad1c.com/dx4win/#Country

The revision history is at the end (or you can get to it from the web 
site).  As always, if there are any corrections or additions, please let me 
know.
