[Dx4win] Work starting on Release 11 of DX4WIN.CTY

A few errors cropped up in DX4WIN.CTY Release 10 and some new info is being 
added.  Here are the changes so far:
    * Added mappings for AC0H, AC1H, AC2H, AC6H, AC7H, AC8H, AC8PN and AC9H 
to United States, K starting 1 January 1976. These calls were used by Gus 
Browning in 1965 from Bhutan, A5
    * FR0CIW/J in September 1980 and FR7AM/E in January 1970 are both Juan 
de Nova; Europa, FR/J
    * SV5FRI/1 in December 2004 is Greece, SV
    * VE2/VE3JM and VE2/VE3EY starting 13 January 2005 are both in Canada, 
VE (CQ Zone 2)
    * Changed start date for VK9XK on Christmas Island, VK9X to 17 
September 1971
    * Changed start date for VU9KV in Andaman & Nicobar Is. to 14 April 1971
    * ZD5F in 1970 is Swaziland, 3DA
There is a new problem with VP8BZL in 1994, I am working to get the 
corrected information.

Please send me any additions or corrections ASAP.  I would like to finalize 
on this so Paul can include it in the 6.04 release.
