[Dx4win] DX4WIN_PACKET_KAM_COMMUNICATIONS?

I am using a KAM "all mode" TNC with DX4WIN for my packet spots off of my 2 meter rig. 
I'm confused as to what I need to do to send a message to the TNC or the packet cluster??? Many times when I do  "sh/dx/10" to get the last 10 spots the KAM comes back with ($   EH  cmd:) and nothing happens? 
I believe I am in the (talk to KAM) mode??? 
I do see a CMD: and this seems to be asking for a TNC command.
How do I tell DX4WIN in the lower part of the "packet window" when I want to send a message to the packet or the TNC itself?
I also noted that if I press "control-C" or "control-D" that I get a "immediate" at the top of the packet window. What is the meaning of this?
I'd like to go between the two "talk to" modes when I want to.
I couldn't find anything in the manual that explains this.
