[Dx4win] DX4WIN.CTY Release 5 available

After working 20+ hours, I finally have a new DX4WIN.CTY file.  You can 
download it from:

Thanks to everyone who contributed information to help improve the database.

I do not want lots of .CSV files this time.  Please let me know ONLY if 
there is there is an error in the database that you can confirm with a QSL 
card.  Please send me all relevant information: callsign as printed on the 
card, date and entity name, and other information if available like CQ 
zone, oblast and/or IOTA.

Note that I am NOT inclined to list large numbers of USA stations in the 
database because of CQ zone errors.  If you fill in the state for these 
QSOs, the errors will go away the next time.
