It was pretty trivial.
My DX4WIN log contains QSOs for two calls - my old WA1SPT call and my 
current K1MU call. I submitted a certificate request for my K1MU 
callsign and got the postcard - then signed a cert request for the old 
call. The valid date range for the old call request (WA1SPT) gave the 
range for that call, ending when my vanity call was submitted.

I dumped my log to ADIF format - from DX4WIN, use "File, 
Import/Export", then make sure the ADIF format is selected.  Then, 
File, Export from the "Import/Export Filters" window. Give it a file 
name for the export.

Run TQSL, then select "File, Sign existing ADIF or Cabrillo File". 
Select the station location for the call you're submitting, then OK. 
Pick the file to sign (the adif file you generated above), then click 
open.  Pick a filename for the output (something.tq8) and click Save. 
That will read the ADIF file and generate a file for you to upload to 
the LOTW site.

Since I have two calls in my DX4WIN log but the certificates have the 
right valid range, I didn't have to do anything to separate the two 
calls - TQSL ignored the out of range QSOs for the two calls - I signed 
the single log two times, fixed the problems it complained about, then 
uploaded the two tq8 files onto the LOTW site.

I also requested a separate callsign cert for my operations from 
Guernsey (GU0/WA1SPT) and submitted those - pretty painless once I had 
the original certificate.

Make sure you've got TQSL 1.8 - there's some bugs in earlier releases 
that cause rejection of the password you're using to protect your 
private key.
