[Dx4win] FOLLOWUP: QSL Labels and Avery 8160 Labels

I finally resolved this problem by deleting and reinstalling the
printer driver for the HP Deskjet 990Cse. The default Avery
8160 format then worked for both Word and DX4WIN!
