Info received last month from N2OO regarding FT5ZM donors and dates:

FWIW, I requested my QSL by OQRS on 2/13, and included a $25 donation, but so far I've not received any LoTW confirmations. I've seen reports of others receiving LoTW confirmations about 2 weeks ago.
