[Dx-qsl] This will be my first submission via a card checker

Will be attending W9DXCC and taking advantage of the card checking at 
the event.
First time I've ever used a card checker.

I've made out the same forms as if I was mailing in the submission.
Approximately 15 qsos on 14 cards.
How do I pay the fees?
for both the checkers (cash I'm assuming)
and to ARRL.

In the past I always filled in my Visa numbers on the application form 
that I mailed to ARRL/DXCC Desk.
