[Dx-qsl] XR0Y OQRS

I am trying to figure out how the OQRS works for the XR0Y operation.  For TY1MS and K4M, they had a "form" to fill out where you entered your contacts;  with the final step contributing via PayPal.
 
I have checked several times on the XR0Y web page, and I have not found any place to submit my contacts for OQRS.  Am I missing something or what?

    Does anyone know how long this ban will be in effect?  I realise 
it is up to the government/leader there, and I vaguely recall the 
explanation when it happened, but can't remember the reason.  Is it 
one of these "until further notice" things?
