[Dx-qsl] Template for Blank QSL Label?

I have a need to print blank (i.e., fillable) QSL labels for my QSL
cards, which don't have one as I print directly on the cards!

I use Avery 5261 labels (1" x 4").

Does anyone have a temple for MS Word, WordPerfect or OpenOffice I can use?
